1. Financial & Insurance Information
Onyx Chiropractic & Wellness, Arbin Mardirosian, Inc., currently operates as a cash-based practice and does not bill or accept insurance plans. Upon request, a Superbill may be provided for services rendered. Submission of the Superbill to any insurance carrier is the client’s responsibility. Any reimbursement, full or partial, is determined solely by the client’s insurance plan. The practice does not guarantee reimbursement and does not influence coverage determinations or payment outcomes. The mobile service is not available to Medicare beneficiaries.

2. Payment Authorization
All major credit and debit cards will be accepted for payment. Payment is due at the time of booking. By scheduling an appointment, the client authorizes Onyx Chiropractic & Wellness, Arbin Mardirosian, Inc., to charge the provided credit or debit card for the full session fee. Payment is processed through a secure, PCI-compliant system and may be stored only if the client elects to save the card for future use.

3. Cancellation and Rescheduling Policy
Appointments may be cancelled or rescheduled without penalty if notice is provided at least 12 hours before the scheduled time. Cancellations or reschedules less than 12 hours prior, failure to provide access, or an unsafe treatment environment will result in a 50% session fee. If the client is not ready at the scheduled time, the session may be shortened or rescheduled, and the 50% fee may still apply. If the provider cannot attend due to unforeseen circumstances, the appointment will be rescheduled or a full refund issued if rescheduling is not possible or if the client elects not to receive care.
